The 2021-2026 Meeker-McLeod-Sibley Community Health Improvement Plan (CHIP) is a long-term plan for the Meeker-McLeod-Sibley Community Health Board (CHB) and local partners to address the top health priorities in our communities as identified in our Community Health Assessment (CHA).

Statewide Health Improvement Partnership (SHIP)
The Statewide Health Improvement Partnership (SHIP), an integral part of Minnesota's nation-leading 2008 health reform law, strives to help Minnesotans lead longer, healthier lives by preventing the chronic disease risk factors of tobacco use and exposure, poor nutrition and physical inactivity. SHIP seeks to create sustainable, systemic changes in schools, work sites, communities and health care organizations that make it easier for Minnesotans to incorporate healthy behaviors into their daily lives.

Meeker Area Partners in Prevention (MAPP)
The mission of Meeker Area Partners in Prevention is to reduce substance use/abuse among youth in Meeker County and over time among all adults through strategies that support policies and community norms that reduce commercial access, social availability and acceptability of alcohol, tobacco and other drugs. MAPP will do this by establishing and strengthening collaborates among community partners and within the coalition. Please visit MAPP's website for more information.
