Children’s Mental Health (CMH) Case Management Services
This is a voluntary service provided by Meeker County Health and Human Services. Children and their families meet with the case manager face to face every month to work on goals they develop together.
To be eligible for Children’s Mental Health Case Management Services, a child must meet the criteria for "Child with severe emotional disturbance", Minnesota Statute 245.4871 Subdivision 6.
When a child is found eligible, CMH case managers assist families in accessing and coordinating a variety of mental health services. These may include individual or group therapy, in-home therapy or skills work (when available), psychiatric care and medication management, Occupational Therapy (OT), special education services within the school setting, and—when available—respite care.
Children’s Mental Health services are designed as short-term supports to help families facing difficulties related to a child's mental health connect with appropriate services and community resources.
CMH social workers also help families build a personalized safety network—a group of trusted individuals who care deeply for the child and are ready to step in during times of crisis. This ensures a clear, collaborative response plan is in place when challenges arise.
Depending on the child’s needs, CMH case managers can also help explore options such as day treatment, intensive outpatient care, or partial hospitalization. If safety concerns require it, they can guide families through the process of accessing residential treatment—always prioritizing the least restrictive options first.
Requesting CMH Case Management Services:
Referrals/Requests for CMH case management can be made directly to MCHHS (See Below) Parent approval for the service along with a copy of the child’s Diagnostic Assessment (DA) that is less than 6 months old that includes a SED (Serious Emotional Disturbance) statement.
