Starting January 1, 2026, PrimeWest Health will be the only Medical Assistance health plan for members under age 65 in our area.
✔️ If you already have PrimeWest, nothing will change
✔️ If you have Blue Plus, you will be automatically moved to PrimeWest
✔️ No action is needed from you
Member ID cards will arrive in early January, and PrimeWest will be available to help answer questions.

Important Health Insurance Change for Certain Kinds of Medical Assistance Members
The Minnesota Department of Human Services (DHS) has told counties that starting January 1, 2026, PrimeWest Health will be the only health plan for Medical Assistance members under age 65 in the 24-county PrimeWest service area, including Meeker County.
This change was expected in the future, but it is happening sooner than planned. Because of this, there has been less time to share information with members and staff.
Blue Plus and PrimeWest are working together to make this change as smooth as possible.
What This Means for You
- If you already have PrimeWest, nothing will change.
- If you are under 65 and have Blue Plus, you will be automatically moved to PrimeWest on January 1, 2026.
- You may get a letter saying you were disenrolled from Blue Plus.
- This is expected. The State will automatically enroll you in PrimeWest.
Important Dates
PMAP Change Timeline
- Member enrollment changes will happen on December 26, 2025.
- Starting December 29, new members can call PrimeWest with questions about care or services.
Member ID Cards
- PrimeWest ID cards will arrive in the first or second week of January.
- If a provider needs to check your coverage before you get your card, they can look you up in the State system.
Member Support
- For questions or help, call the PrimeWest Member Contact Center at 1-866-431-0801.
- You will speak with a live person.
- The PrimeWest call center will be open on January 1, 2026.
- PrimeWest has already contacted pharmacies and transportation providers in the region to prepare for this change.
Care and Member Information
- DHS will share member information with PrimeWest to help keep care going without interruption.
- If you have concerns about your care or information, call the PrimeWest Member Contact Center.
Transportation and Rides
Transportation Call Center: 1-888-814-6643
- If you have a ride scheduled and need transportation after January 1:
- Call PrimeWest first before canceling any BlueRide trips.
- PrimeWest will work with the transportation provider to see if the ride can be transferred.
- Anyone needing rides in the first week or two of January should call PrimeWest for help.
Provider Network Questions
- If your doctor or provider is not in the PrimeWest network:
- Call the PrimeWest Member Contact Center.
- PrimeWest will contact the provider to talk about care options.
Final Notes
We know this change may raise questions. PrimeWest has added more customer service staff and extended availability to help members during this transition.

Vulnerable Adult Maltreatment
All reports of Maltreatment of a Vulnerable Adult need to be reported to the Minnesota Adult Abuse Reporting Center (MAARC). Toll free at 1-844-880-1574 or by clicking the heading to be re-directed to the online form. Maltreatment can include self neglect, care giver neglect, emotional/mental abuse, physical abuse, sexual abuse and financial exploitation.

Southwestern Minnesota Adult Mental Health Consortium
SMAMHC is one of the 19 Adult Mental Health Initiatives in Minnesota and is an 18-county Joint Power entity. The counties in our region include Big Stone, Chippewa, Cottonwood, Jackson, Kandiyohi, Lac qui Parle, Lincoln, Lyon, McLeod, Meeker, Murray, Nobles, Pipestone, Redwood, Renville, Rock, Swift, and Yellow Medicine. SMAMHC is committed to the partnerships and collaboration built with the Counties and Providers in the Southwestern 18-county region. We work together to inspire, provide hope, empowerment, and overall wellness. The SMAMHC is dedicated to improving the mental health of our region through these strong partnerships and planning. Programs and services we support in our region include:
- Core Adult Mental Health Services: Assertive Community Treatment, Adult Rehabilitative Mental Health Services, Intensive Residential Treatment, Crisis Stabilization
- Housing Vouchers: Rental assistance to help maintain community housing; support to West Bridge, a specialized Board and Lodge located in Marshall.
- Flexible Funds: Limited discretionary funds to meet individual needs of consumers (medication, housing, vocational and general needs)
The Southwestern Minnesota Adult Mental Health Consortium has monthly meetings held on the 2nd Friday of the month starting at 9:30 a.m. The following is a list of dates for 2024:
January 12th, February 9th, March 8th, April 12th, May 10th, June 14th, July 12th, August 9th, September 13th, October 11th *Note this is our Annual Meeting held at the Redwood Area Community Center in Redwood, MN), November 8th, December 13th.

Meeker County LAC (Local Advisory Council)
The Meeker County LAC (Local Advisory Council) is a group of people who discuss and explore adult mental health issues and concerns in our community. The LAC assists in educating the public, informing people of available services, and gathering input and concerns of mental health needs. The LAC makes recommendations to the Meeker County Board of Commissioners to improve mental health services and meet needs.
In the fall of 2020, the Meeker LAC joined with the Meeker Cares Community Leadership Team to discuss and identify the mental health needs of members of Meeker County. Meeker Cares' goal is to provide greater well-being, create awareness of trusted community/available resources, and get information of those resources out to members of Meeker County. Meeker LAC and Meeker Cares also collaborate in creating resources for residents. The Meeker Cares resource page is located on this website; click Meeker Cares on the banner at the top of this page.
